How to create a form on V4 Builder:
1) Select "Forms" tab on the top of builder
2) Click "Add Form" bottom right corner
3) Here in the "General" tab you would name your form and select basic options for your customers
4) Within the "Elements" tab you can add Form elements. Next to "Element to add" use the drop down to pick which type of Element to add, then click the "Add Form Element" to complete.
5) Within "Notification" tab you can enter your email or fax
6) Within "Confirmation" tab you will enter the text customer will receive once they complete your form.
7)Within "Scripts" you can add code for advanced users.
How to add Form to page on V4 Builder:
1) Select "Pages" Tab at the top of Builder
2) Click "Edit" on the page you wish to link the Form to
3) Choose "Link" within the drop down next to "Element to Add" and click "Add Element"
4) Within new Link element line click "Edit"
5) Add text you wish to link
6) Under "Link to" select "Form" and click the newly created form
