How to Add A Decision Form Element to Order Form:

1) Under "Site" Menu on left side of builder

2) Select "Forms"

3) Double Click on the "Order Form"

4) Select "Elements" tab on the right side of the builder

5) Click the "+" icon at the bottom of the builder

6) Hover over "Input" and Select "Decision"

7) Here you can add Heading, instructions and decisions (with or without pricing) by clicking the "+" Icon at the bottom
