How to add a form to page:

1) Under "Site" on left side of Builder

2) Select  "Pages"

3) Double click on the page you wish to add Form to 

4) Select "Elements" on the right side of Builder

5) Click "+" icon at the bottom of the Builder

6) Hover over "Product" and Select "Form"

7) Here you can select the form you created.

The entries into this form will go to the email you added to "notifications" and into your orders section under "Fullfillment"​
